For me this song is about couple who started well together and went off track in the end. the guy in the song did a lot of wrongs and the first person is the girl asking the guy if he will still continue what his doing and lose their relationship or not.

-Drive until you lose the road
Or break with the ones you've followed-

Saving a life not in literal terms but their life together. it can also be about the parent asking his son to stop his destructive ways, either drugs or bad things... she doesn't know how to save his life means she doesn't know what to do anymore. Nobody got killed in the song at least literally. but people can die without going to the grave.

The song is about a relationship gone bad and the sad reality of ending a life (metaphorically ) .

Alternately my GF thinks it is about a relationship gone bad and aborting an unwanted pregnancy
